This notice is intended to inform you that the credit insurance coverage on your loans and credit cards is being provided by a new carrier, Minnesota Life Insurance Company, 400 Robert Street North, St. Paul, Minnesota 55101 To show your acceptance of the Minnesota Life plan, simply continue making your loan payment which includes your credit insurance premium. The Minnesota Life plan is optional and you may terminate your coverage at any time. If you previously elected, and are currently paying premium for credit insurance, please review the attached certificate carefully as claims will be administered according to the provisions stated in the certificate.
